Mechanism of Action:
The definite mechanism of action of Pregabalin has not been described. However, there is evidence that this drug acts by binding to α2δ subunit of voltage-dependent calcium channels, preventing influx of calcium ion in the pre-synaptic neuron. It prevents membrane depolarization and release of excitatory neurotransmitters to the synapse hence reducing neuronal firing and demonstrating its efficacy in epilepsy, GAD and neuropathic pain. Methylcobalamin has been found to be important in maintaining and regenerating peripheral nerves and promoting the process of myelination, leading to functional restoration of myelin sheath. It has also been postulated that B12 could promote nerve regeneration by up-regulating gene transcription and decrease ectopic nerve firing, explaining why it might help to reduce painful symptoms.
Indications:
The combination is used in the management of neuropathic pain (Diabetic neuropathy, Peripheral neuropathy, post-herpetic neuralgia, trigeminal neuralgia), fibromyalgia.
Dosage:
The dose range for pregabalin is 150 to 600 mg per day given in either two or three divided doses. For neuropathic pain, pregabalin can be started at a dose of 150 mg per day given as two or three divided doses. Based on the response and tolerability, the dose may be increased to 300 mg per day after an interval of 3 to 7 days, and if needed, to a maximum dose of 600 mg per day after an additional 7-day interval.
