Composition: Mupirocin 2%

Therapeutic Category: Topical antibiotic

Mechanism of Action: Mupirocin 2% ointment works by inhibiting bacterial protein synthesis through reversible binding to isoleucyl-tRNA synthetase, an enzyme essential for incorporating isoleucine into bacterial proteins. This leads to disrupted protein production, resulting in bacteriostatic effects at low concentrations and bactericidal effects at higher concentrations. It is primarily effective against Gram-positive bacteria, including Staphylococcus aureus (including MRSA) and Streptococcus pyogenes. Due to poor penetration, it has limited activity against Gram-negative bacteria.

Indications:

1. Primary Skin Infections

Impetigo – A contagious bacterial skin infection, often seen in children, characterized by honey-colored crusts.

Folliculitis – Infection of hair follicles leading to small, red, pus-filled bumps.

Ecthyma – A deeper form of impetigo that causes ulcerative lesions.

2. Secondary Skin Infections

Infected cuts, wounds, abrasions, and burns – Helps prevent the spread of infection in minor injuries.

Infected dermatitis – Used when conditions like eczema or psoriasis become secondarily infected.

Infected insect bites – To prevent bacterial colonization and further complications

Dosage:

1. Adults and Children (3 months and older)

Dosage: Apply a small amount (a thin layer) of mupirocin 2% ointment to the affected area 3 times daily.

Duration: Typically 5–10 days, depending on the severity of the infection.

Instructions: The area can be covered with a dressing if necessary.

2. Infants (under 3 months)

The safety and effectiveness of mupirocin in neonates have not been well established. Use should be based on a doctor's prescription and careful assessment.
